In an effort to help you do business with City Hall from home, we have made fillable PDF versions of several forms.

These forms require PDF reader software. The most common is Adobe Reader.

Note that some forms require a signature; you can fill out the document on your computer, but you will need to either:

1. Print the document, sign it, and mail it to our office (55 Front St., Bath, ME 04530).

2. Print the document, sign it, then email a photo or scan of the signed document to our office.

3. Use a digital signature service, such as DocuSign or Adobe, to sign the document, and then email it to our office.


FOR HOMEOWNERS

Permit Update Fill-In Form  - Use this form to help update the Assessor's record of a property when a project requiring a building permit is complete or partially complete on April 1st. Due by June 1. 

Homestead Application.pdf  - Due by April 1 of the tax year you are applying for. Once you have the homestead exemption, you do not need to reapply unless you have moved.  

Veteran Exemption Application.pdf  - NOTE: Additional materials are required. Leave SSN/Service ID blank if submitting by email. Staff will contact you for follow-up. Due by April 1. Once you have the veteran's exemption, you do not need to reapply unless you have moved. 

Property Tax Stabilization for Senior Citizens is a State program that allows certain residents over age 65 to stabilize, or freeze, the property taxes on their homestead. As long as you qualify and file a timely application each year, the tax billed to you for your homestead will be frozen at the amount you were billed in the tax year of your original application. Eligible residents who move may transfer the fixed tax amount to a new homestead, even if that new homestead is in a different Maine municipality. Note: Additional materials may be required to be included with your application. Due by December 1 each year.

RENEWABLE ENERGY

Application for Renewable Energy Equipment Exemption (Fillable).pdf  - Detailed information from equipment installer is required. Due by April 1 in the tax year you are applying for. Once you have the solar exemption, you do not need to reapply unless requested by the assessor or if something has changed about your solar equipment.
