Death Certificate

REQUEST A CERTIFIED (LEGAL) COPY
OF A DEATH CERTIFICATE

You may request a certified death certificate by visiting the City Clerk’s Office on the First Floor of City Hall or by writing us a letter including the following information:

1) Full name of the deceased, including first, middle and last name
2) Date of death
3) Your relationship to the deceased
4) Name and mailing address where we should send the certificates
5) Number of certified copies you are ordering

(It is $10.00 for the first copy and $5.00 for each additional copy of the same record, ordered at the same time.)

Please make check or money order payable to "City of Bath"and make sure your telephone number is on the check.
