 REQUEST A CERTIFIED (LEGAL) COPY
OF A DEATH CERTIFICATE

You may request a certified death certificate by visiting the City Clerk’s Office on the First Floor of City Hall or by application:

STARTING JULY 12, 2010 FEES WILL BE CHANGING: $15.00 FOR THE FIRST COPY AND $6.00 FOR EACH ADDITONAL COPY OF THE SAME RECORD, ORDERED AT THE SAME TIME.

Please make check or money order payable to "City of Bath"and make sure your telephone number is on the check.
