Birth Certificates

REQUEST A CERTIFIED (LEGAL) COPY
OF A BIRTH CERTIFICATE

You may request a certified birth certificate by visiting the City Clerk’s Office on the First Floor of City Hall or by writing us a letter including the following information:

1) Full name of the child, including first, middle and last name
2) Date of birth
3) Child's Mother's first, middle and maiden names
4) Child's Father's first, middle and last names
5) Your relationship to the child named on the birth certificate
6) Name and mailing address where we should send the certificates
7) Number of certified copies you are ordering

(It is $10.00 for the first copy and $5.00 for each additional copy of the same record, ordered at the same time.)

Please make check or money order payable to “City of Bath” and make sure your telephone number is on the check.
